The Thought Record: A CBT Worksheet for Challenging Unhelpful Thoughts

A step-by-step CBT worksheet to catch a distressing thought, examine the evidence, and build a more balanced way of seeing the situation.

The thought record is one of the most well-studied tools in cognitive behavioral therapy. It doesn’t try to talk you out of a feeling — it slows down the automatic thought behind the feeling long enough for you to actually look at it, which is usually enough to loosen its grip a little.

Use it any time a thought is looping, a feeling has spiked, or you notice yourself already assuming the worst about how something will go.

How to use it

Work through the six steps below for one specific moment — not “I’m anxious in general,” but the actual situation that triggered it.

1. Situation. What was actually happening? Just the facts — where you were, what was said, what you were doing. No interpretation yet.

2. Automatic thought. What went through your mind in that moment? Write the exact thought, as bluntly as it appeared — “I’m going to get fired,” not “I was worried about work.”

3. Emotion, and how strong (0–100%). Name the feeling (anxious, ashamed, angry, sad) and rate how intense it was right then.

4. Evidence for the thought. What actually supports it being true? Stick to facts a neutral observer would agree with, not other anxious thoughts.

5. Evidence against the thought. What doesn’t fit? Past experience, things you’re forgetting under stress, alternative explanations.

6. Balanced thought, and re-rate the emotion. Given both sides, what’s a more accurate way to see it? Then rate the original emotion again — it usually drops, even if it doesn’t disappear.

Why it works

Distressing thoughts feel like facts in the moment — that’s what makes them distressing. Writing out the evidence on both sides is what separates a thought from a fact, and that gap is where the anxiety usually loses some of its power. This isn’t about forcing positivity; a balanced thought can still be uncomfortable, it just has to actually fit the evidence.
